Timesheet

Timesheets must clearly state your name, show the time you reported for an assignment, the time you took off for a meal, any other time taken away from the assignment during work hours, as well as your completed work hours for the day.  Both your signature and the signature of your supervisor are required. The absence of one or both signatures may delay the processing of your paycheck until the following week. A timesheet must be completed for each assignment worked during that week. You may scan and email the completed timesheet to info@onlineits.com no later that 12:00 pm on Monday. If following Monday is a legal holiday, the payroll deadline is midnight on that Friday. Timesheets received after the deadline will not be processed until the following week.

Direct Deposit

To enroll in direct deposit, please download the PDF, print and fill it out, then scan and email it to info@onlineits.com. Once you sign up for direct deposit, it will go into effect after two paychecks.

W-4 & NC-4

To change withholding allowances, please download the PDF, print and fill it out, then scan and email it to info@onlineits.com.

I-9

We are required to complete and retain a Form I-9 for every employee we hire. The list of acceptable documents include a valid U.S. Passport or a valid driver’s license or ID card issued by a State and a Social Security Account Number card. Additional acceptable documents can be found on the uscis.gov website.

Name and address changes must be submitted in writing to info@onlineits.com and should include your name, social security number, new address, signature and effective date. No changes will be accepted over the phone.
